We serve more than 5,000 adults, Veterans, families, and youth each year through a continuum of care, which meets people where they are and works to support a housing plan specific to each individual.

Introducing The SCC Homelessness Prevention System (HPS)!

The Santa Clara County Homelessness Prevention System (HPS) is a countywide network of agencies working together to prevent homelessness. 

As a member of this network, HomeFirst believes that homelessness prevention is key to maintaining healthy, safe communities.

HomeFirst's outreach team provides assistance to un-housed individuals looking to connect to support services. Our team can help find emergency shelter, showers, laundry, meals, medical services, case management, and more!
