Outreach
HomeFirst has the largest outreach program for people living on the streets and in encampments in San Jose. Each day, our team distributes necessary supplies and information to those who are unsheltered within the city of San Jose and helps to resolve issues that prevent individuals from getting housed.
TOEP - Targeted Outreach Engagement Program
TOEP, HomeFirst’s Targeted Outreach Engagement Program offers intensive case management to people living in four designated encampments across the city of San Jose. The 6-month program helps people find housing opportunities and enables the city to return the inhabited space to its intended use. The TOEP Team offers individuals snacks, hygiene kits, and transportation. Staff help people experiencing homelessness sign up for benefits, register their vehicles, and access needed medical care. They also connect individuals with the Here4You Hotline and provide housing referrals.
Citywide Team
Citywide is HomeFirst’s reactive outreach team that responds to community concerns, which are voiced through the City of San Jose’s website. The team offers light-touch case management to people experiencing homelessness across the city. The Citywide Team offers individuals snacks, hygiene kits, and transportation. Staff help people experiencing homelessness sign up for benefits, register their vehicles, and access needed medical care. They also connect individuals with the Here4You Hotline and provide housing referrals.
Valley Water Program
HomeFirst’s Valley Water Program offers intensive case management to pre-identified people living at pre-identified locations along the waterway at Coyote Creek, an area prone to flooding. The Valley Water Team offers individuals snacks, hygiene kits, and transportation. Staff help people experiencing homelessness sign up for benefits, register their vehicles, and access needed medical care. They also connect individuals with the Here4You Hotline and provide housing referrals.
