Permanent Housing

HomeFirst offers two types of Permanent Housing Support - through property management, and case management. The individuals we serve through our Permanent Supportive Housing programs live in apartments. They receive subsidized housing vouchers from the Office of Supportive Housing (OSH) and on-site case management from HomeFirst.

Property Management

HomeFirst serves as the property manager at both the Sobrato House and the Boccardo Family Living Center. These are apartment complexes that provide affordable housing to families who were formerly homeless or at risk of becoming homeless.

Boccardo Family Living Center (BFLC)

HomeFirst serves as the property manager at the Boccardo Family Living Center. This is an affordable housing complex in San Jose that caters to families that have experienced housing insecurity.

Sobrato Family Living Center (SFLC)

HomeFirst serves as the property manager at Sobrato House, an affordable housing complex in San Jose. Sobrato House serves youth ages 18 to 25, some of which have children. These young adults have transitioned out of foster care and experienced housing insecurity.

Case Management

HomeFirst offers case management to a portion of the tenants living at three affordable housing complexes in San Jose, namely Mesa Terrace, Iamesi Village, and Markham plaza. The individuals that we serve need both financial and social assistance, and as such, receive subsidized housing vouchers from the Office of Supportive Housing (OSH).

Iamesi Village

HomeFirst provides case management services at Iamesi Village, an affordable housing development in San Jose that is owned by First Community Housing. HomeFirst provides case management to each of the 60 residents living in the 60 of 135 affordable housing units that are designated for individuals with a history of chronic homelessness.

Mesa Terrace

HomeFirst is responsible for running the Transition Aged Youth Program (TAY) at Mesa Terrace, a recently opened, affordable housing complex in San Jose. HomeFirst is providing case management to each of the 23 tenants living in the 23 of 46 affordable housing units that have been designated for transition aged youth. These are young adults between the ages of 18 and 25 that could benefit from financial and social assistance - some were previously homeless, some were formerly a part of the foster care system, and others were recently released from jail.

Markham Plaza

HomeFirst provides case management services at Markham Plaza, an affordable housing development in San Jose. HomeFirst provides case management to each of the 60 residents living in the 60 of 200+ housing units that are designated for individuals with a history of chronic homelessness.

Solaire Apartments

The Solaire Apartments is a 130-unit apartment complex that includes 64 units specifically allocated for individuals and families transitioning from homelessness that have project-based housing vouchers. In support of these residents, HomeFirst offers intensive case management. We aim to facilitate a smooth transition from chronic homelessness to stable housing and provide ongoing support to help residents sustain tenancy. HomeFirst Case Managers and a Resident Services Coordinator work on-site, and these dedicated employees empower residents to connect with resources, increase their income, and achieve other goals they set.
